
First Visit
Your first visit will typically include a consultation to explain your diagnosis and discuss treatment options. In most cases, treatment can be performed on the same day. However, patients with complex medical histories may require a separate evaluation.
To help us serve you efficiently, please bring the following:
- A referral card and any X-rays from your dentist.
- A list of any medications you are currently taking.
- Completed insurance forms, if applicable.
IMPORTANT: A parent/guardian must accompany all patients under 18.

Scheduling
Monday through Friday – Please call for current hours.
- We make every effort to schedule your appointment promptly. For emergencies, we’ll do our best to see you the same day.
- After-Hours Emergencies: Please leave a detailed voicemail, and we’ll return your call.

Financial Policy
For your convenience, we accept Visa, MasterCard, American Express, Discover, and CareCredit. We are committed to providing the highest quality of care at a reasonable cost; therefore, payment is due at the time services are rendered, unless prior arrangements have been made.
We accept assignment of dental benefits from most insurance companies. However, please remember that you are fully responsible for all fees charged by this office, regardless of your insurance coverage.
